the documents for establishing a representative office in shanghai:
(一) 申請書 (由公司董事長或總經(jīng)理簽名)
application letter (signed by chairman of the foreign enterprise)
申請書內(nèi)容如下:
content as follow:
1)常駐代表機構(gòu)的名稱/ the name of representative office;
2)首席代表機構(gòu)人員名單（首席代表、代表）/ list of chief representative and representatives;
3)企業(yè)簡況,設(shè)立常駐代表機構(gòu)的目的.業(yè)務(wù)范圍/ brief introduction of the foreign enterprise, purpose of establishing this representative office, business scope of this representative office,
4)辦公地址/ office address
5)駐在期限/ residence period
(二) 代表任命書 (由公司董事長或執(zhí)行董事簽名)
application of chief representative and representatives (signed by chairman of the foreign enterprise);
如董事長任首席代表或代表，其授權(quán)書必須由該企業(yè)董事會兩名以上董事簽署
if chairman is also the representative of the office, the application has to be signed by more than two directors
(三) 代表的簡歷
resumes of representatives；
(四) 代表護照或身份證復印件
the passport or id card (copy) of the chief representative and representatives;
(五) 由該企業(yè)所在國的有關(guān)當局出具的開業(yè)合法證書（副本）注明有效期
business license copy;
(六) 將此證書在當?shù)氐闹袊笫桂^/領(lǐng)事館公證。
notarizing this certification at the china embassy /consulate in local;
(七) 由同該企業(yè)有往來的銀行出具的資信證明書（正本）注明信用評價及有效期
credit certification (original) issued by a foreign bank which has business relations with the enterprise;
(八) 辦公樓租賃合同（原件）
contract for lease of house (original);
(九) 房屋出租方提供的材料
documents provided by lessor

1)房屋產(chǎn)權(quán)證（復印件）
muniment of title (copy)
2)外經(jīng)委批準涉外辦公的文件(復印件)
official document by shanghai foreign economic relation & trade commission which permitting the house used by foreign company （copy）
3)由物業(yè)出具的住在證明
the certificate by the building estate management department
(十) 首席代表和代表各需4張2寸照片
four 2-inch-photos of chief representative and representatives;
(十一) 委托本公司代理辦理的委托書
